RESPONDING TO REQUESTS FOR DISCLOSURE OF PERSONAL INFORMATION

Please note that all procedures and documents are including mailing, fees, and forms are only in Japanese. Please let us know if you are requesting from overseas.

Based on Article 37 of the Act on the Protection of Personal Information (hereinafter referred to as the "Act"), we do not accept requests for disclosure, correction, etc., suspension of use, etc. from individuals or their agents regarding their "retained personal data". Regarding requests for notification of purpose of use (hereinafter referred to as "requests for disclosure"), we have stipulated the following.

Identification of “Retained Personal Data”

When making a request for disclosure, please specify as much as possible the "Retained Personal Data" in question based on the content and circumstances of the information being used by our company. Please note that your information will not be subject to disclosure if the following (1) or (2) is applied.

(1) Cases that do not fall under “Retained Personal Data”

Data that we do not have the authority to disclose, such as data for which we are simply entrusted with data processing by a third party. Items specified by the Law Enforcement Order as ones that would harm the public interest or other interests if the existence or non-existence of such items becomes clear.

(2) In cases where information is not subject to disclosure pursuant to the provisions of Article 33, Paragraph 2 of the Act.

When disclosing the information may harm the life, body, property, or other rights and interests of the person or a third party.

If disclosure is likely to significantly impede the proper implementation of our business.

If disclosure would violate other laws and regulations

Scope of Personal Information Subject to Disclosure

The scope of personal information subject to disclosure is limited to "Retained Personal Data" collected and currently held by our company, such as the person's name and address, and the purpose of use by our company.

Request Disclosure

When making a request for disclosure, please send the Request Form with necessary documents by mail to address below. The necessary documents include identity verification and the disclosure fee (Only in the case of a request for disclosure or notification of purpose. Fees are not required in the case of corrections and suspension of use.) Please note that we cannot accept requests from those who come directly to our office.

Request Form and Documents

If you wish to make a request for disclosure, etc., please download the following Request Form (A), print, fill in the specified information, sign, and submit with Documents for Identity Verification (B) . Please note that if the request is made by a representative, the documents listed in 5 below will be required.

*1 All miscellaneous expenses required for the application for disclosure, such as printing/copying costs, costs for obtaining identity verification documents, postage costs, etc., will be borne by the individual. If any other actual expenses are required, we will bill you separately.

*2 Request documents must be those specified by our company, and no other formats will be accepted.

*3 Please note that we will not return any documents submitted when making a request for disclosure.

*If your certificate includes your Permanent Domicile, please cover the "Permanent Domicile" part by blacking it out or pasting it with paper before making a copy.
*If there is no expiration date displayed, the certificate must be valid within 6 months of issue.

Request for Disclosure by Representatives

If the legal representative or an agent delegated by the person is making a request, add following documents for (C) or (D) to the documents described in section 4.

*If your certificate includes your Permanent Domicile, please cover the "Permanent Domicile" part by blacking it out or pasting it with paper before making a copy.

(C) In the case of a legal representative

■ In the case of a legal representative of a minor
1. Documents to confirm that you have legal power of representation.
① If the person has parental authority
…One copy of the person's family register
② In the case of guardian of a minor
…One copy of the person's family register extract
2. One set of Documents for Identity Verification described in 4(B)

■ In the case of a legal representative of an adult ward
3. One copy of Certificate of registered matters (Certificate of registered matters stipulated in Article 10 of the Act on Guardianship Registration, etc.)

(D) In the case of a delegated agent

1. Power of Attorney

*The personal seal certificate must be issued within 6 months of issue.

Fees and Payment Methods

■ In the case of requests for disclosure and notification of purpose of use

Fee: 624yen per request (including consumption tax and local consumption tax)

Please note that when requesting disclosure and notification of purpose of use, we will limit up to four pieces of "Retained Personal Data" per request.

Payment Method: Please enclose 624yen Postage Stamps (We only accept Japanese postage stamps) with your request document.

*The above fee is equivalent to the postage charge for registered mail (Person-only delivery mail). Please be aware in advance that if postage rates change, the fees for requests for disclosure will also change.
Postage fee: 84 yen + Registered mail fee: 435 yen + Personal pick-up fee: 105 yen = Total: 624 yen

*If postage costs exceed the above amount due to the disclosure method, or if any other actual expenses are required, the individual will be responsible for paying them separately.

■ In the case of requests for disclosure and notification of purpose of use

No fees will be charged if you request correction or suspension of use after specifying "retained personal data."
*If you are unable to specify the "Retained Personal Data," please first request disclosure and then identify the retained personal data for which you would like to request correction or suspension of use. Please note that in this case, a fee will be charged for the disclosure request.

7.

Deficiencies in Request Documents

If we are unable to proceed with disclosure due to the following reasons such as incomplete documents, we will contact you using the contact information you provided on the request form. Please note that if we do not receive your request again within two weeks of contacting you (including cases where it has not been received), we will treat your request for disclosure as not having been made.

If there are missing, incorrect, or unclear parts in required documents .

If we cannot identify Documents for Identity Verification due to missing documents or unclear parts in copy.

If we cannot confirm the authority of representation due to missing or inadequacy of required documents.

If the fee is insufficient or not enclosed.

If we cannot proceed the procedure due to other deficiencies in the requests.

8.

Respond to Requests for Disclosure

We will respond via documents mailed to the address you provided on your Request Form. We use registered mail (Person-only delivery mail) which is delivered by Japanese Post Co. This service is limited in Japan, and we cannot mail to overseas.

*Whether the requester is a legal representative or a delegated representative, the written response to the request for disclosure (including the written notification in the case of non-disclosure) will be sent to the person to whom the disclosure is made. Please note that the invoice will be mailed to the address you submitted as the recipient.
*Person-only delivery mail is a service provided by Japan Post Co., Ltd. that allows mail to be delivered to only one person, the addressee listed on the mail.

9.

Non-Disclosure of “Retained Personal Data”

Information will not be disclosed in the following cases: If we decide not to disclose the information, we will respond in writing to the address you entered on the request form by registered mail (mail only available to the individual), along with the reason. Please note that a prescribed fee will be charged even in the case of non-disclosure.

If the address stated on the invoice, the address stated on the document for identity verification, and the address registered in our "retained personal data" do not match, we may be unable to confirm that the request is made by the individual. If you cannot confirm
If the authority of representation cannot be confirmed when a request is made by a representative
If the personal information requested for disclosure does not fall under our "retained personal data"
When there is a risk of harming the life, body, property, or other rights and interests of the person or a third party.
In cases where there is a risk of significant hindrance to the proper implementation of our business.
If there is a problem with the requested documents, etc., and you do not submit them again within 2 weeks after contacting us.
In cases where it would violate other laws and regulations.

10.

Purpose of use of personal information obtained in connection with requests for disclosure

Personal information obtained in connection with a request for disclosure. will be handled only to the extent necessary to respond to the request for disclosure. The documents you have submitted for identity verification will be destroyed in an appropriate manner without delay after we have responded to your request for disclosure.
